Synthesised at the Institute of Molecular Genetics, Russian Academy of Sciences. C-terminally extended analogue of tuftsin (Thr-Lys-Pro-Arg) with Pro-Gly-Pro added for metabolic stability. Approved in Russia (2009) as nasal spray for GAD and neurasthenia.
Mechanism
Positive allosteric modulator of GABA-A receptors (upregulates alpha2, alpha3, gamma2 subunits) without occupying the benzodiazepine site. Inhibits aminopeptidase N and DPP-IV, prolonging endogenous enkephalin. Normalises dopamine under stress, increases serotonin turnover, upregulates BDNF in hippocampus and PFC. Immunomodulatory via tuftsin lineage (reduces IL-6, TNF-a).
Outcome
RCT (N=62): matched medazepam on Hamilton Anxiety Scale reduction with additional antiasthenic and mild psychostimulant effects. No tolerance, no withdrawal, no sedation in any published trial. Cognitive improvements in elderly (28-day study).
