In Plain English: A nine-amino-acid peptide your hypothalamus makes naturally and releases during childbirth, breastfeeding, sex, and close social contact. Synthetic oxytocin (Pitocin) has been FDA-approved since the 1960s for obstetric use. Origin
Endogenous nonapeptide; produced in the paraventricular and supraoptic nuclei of the hypothalamus, released from the posterior pituitary. Synthetic form (Pitocin/Syntocinon) produced by solid-phase peptide synthesis.
Mechanism
Binds G-protein-coupled oxytocin receptors (OXTR) in uterine myometrium, breast myoepithelial cells, and CNS limbic structures. Peripheral: raises intracellular Ca²⁺, triggering uterine contractions and milk ejection via positive feedback. Central (intranasal route): delivered trans-nasally via CN-I/CN-V pathways, acts on amygdala, hypothalamus, and nucleus accumbens to enhance social salience, dampen threat appraisal, promote trust, and modulate HPA-axis cortisol release.
Outcome
Obstetric: labour induction and postpartum haemorrhage control (A-grade evidence). CNS: reduced social anxiety and improved facial-emotion recognition in RCTs; mixed results for autism core symptoms; preliminary PTSD fear-extinction facilitation.
