Origin
Endogenous neuropeptide first isolated by Schally and Guillemin (Nobel Prize 1977). Factrel (hydrochloride) FDA-approved 1978 for diagnostic use; Lutrepulse (acetate) for pulsatile ovulation induction. Both discontinued commercially; remains compoundable and widely used off-label.
Mechanism
Binds GnRH receptor type I on pituitary gonadotrophs, coupling Gq/G11 to PLC. IP3 triggers Ca2+ release; DAG activates PKC. Drives LH and FSH synthesis/exocytosis. CRITICAL: pulsatile (q60-120min) sustains secretion; continuous dosing causes receptor downregulation and suppression (Belchetz Paradigm 1978).
Outcome
Diagnostic: 100mcg IV elicits peak LH at 27-71 min. Therapeutic: 70-80% achieve spermatogenesis in 6-24 months (HH patients). Ovulation: 75% rate, 18%/cycle pregnancy rate. TRT adjunct: maintains testicular volume with <1% OHSS risk.
